• Submit your valid signed U.S. passport, must have at least 6 months remaining and one blank visa page
• 1 Fully completed and signed Visa Application Form
• 2 Passport photographs
• The passport and travel documents must not contain evidence of prior or intended travel to Israel
• Covering letter specifying the length of stay and purpose of the trip
• Copy of ticket or travel itinerary
• Applicants of Lebanese origin must present copy of Lebanese ID card
• Spouse of applicants of Lebanese origin must submit a copy of the marriage certificate and a copy of the spouse's Lebanese ID
• Child under age 18 must submit a notarized letter of consent from both parents
• Completed Cover Letter

• Applicants of Lebanese origin must present copy of Lebanese ID card
• A business letter of responsibility (sample) is required, to be typewritten on your company letterhead and addressed to:
• Consulate General Lebanon, Visa Section, The letter must explain all of the following :
• The nature of business to be performed
• Business reference to be visited in Lebanon
• Guarantee financial support for the applicant while in Lebanon
• Return transportation to the U.S.A and a letter or fax from the business contact in Lebanon stating the purpose of the trip
• Completed Cover Letter
